Member of the Civilian Component
You are: Member of the Civilian Component (NATO SOFA) or Consultant hired by SHAPE, or any other Organization supported or administered by SHAPE.

1. Application SHAPE Id: please visit HSG-Central Processing Centre (CEP - Bldg. 210), with a completed
SHAPE Form 94 (SF94), which should be signed by your HR Civilian Personnel Branch or NMR and submitted to Status and Classification Section (STC - Rm.111) for the determination of your legal status and level of entitlements. In addition a copy of your National Id or passport (valid for one year) and a copy of your marital status certificate, common law certificate or cohabitation agreement is requested.
2. Proceed to the Registration Section (RGN- Rm. 110) to complete your registration and receive your SHAPE Id, ration cards and driving license.
3. Registration in Belgium: you are invited to apply for the Belgium Special Id Card (S-ID) released by the Ministry of Foreign Affairs, which is the valid proof of residence in Belgium for special foreigners like yourself.
4. Application Belgian S-ID: please complete the application file for S-ID; for NATO Civilian please make sure to see civilian HRM, Bldg 102; for National Component, please visit your NMR/NSU (National Support Unit). The NMR or NSU will help you with the completion of the application.
Then submit the entire package:

two filled in application forms (must be done using computer, hand written copies will not be accepted)

two copies in colour of the national passport or national ID for each applicant

two copies of the birth certificate for all children stamped and signed by NMR for authentic and original, if those documents are not made in English, French, German our Dutch.

two copies of the marriage certificate or certificate of legal partnership,stamped and signed by NMR for authentic and original, if those documents are not made in English, French, German or Dutch.
to the Registration Section (RGN -Rm. 110). They will take care about your photos and the final submission to the Ministry of Foreign Affairs. The S-ID is for all ages.
