Documents Necessary for SHAPE ID

Sponsor:

SF 94: original completed electronically and signed by either the National Military Representative (NMR) or any other individual nominated by the NMR, and certified by an official NMR stamp.
European National ID/Passport: original, not photocopy
NATO Security Clearance Certificate: original with official NMR stamp.
SF 1: signed/stamped by the Divisional Security Officer (DSO) of the division to which you are assigned.
ACE Form 107 R: signed/stamped by the DSO of the division to which you are assigned.

Dependents:
SF 94: completed electronically, signed and stamped by the sponsor's NMR
European National ID/Passport for each applicant: original, not photocopy
