Employees of any firm which is allowed to operate on the SHAPE installation, by virtue of a contract with SHAPE.

DOCUMENTS NECESSARY FOR SHAPE ID:

SF 94: completed electronically, signed and stamped by your HR Civilian Personnel Department.
NATO Security Clearance Certificate OR HQ 3: stamped and signed by Belgian Federal Police (FSCEP - Bldg. 210).
SF 1: (SHAPE Security Pass Application) - signed, stamped by the responsible Division Security Officer (DSO).
ACE Form 107 R Jul: (Personnel Security Clearance) - signed, stamped by the responsible DSO.
Original Belgian registration document from the commune accompanied by one copy OR Belgian ID
