Documents necessary for SHAPE ID:

SF 94: completed electronically, signed and stamped by your HR Civilian Personnel Department.
Attestation or Certificate of assignment: stamped and sign by Civilian Branch Office.
NATO Security Clearance Certificate: original and certified by an official NMR stamp.
SF 1: signed/stamped by the responsible Divisional Security Officer (DSO) of the division you are assigned to.
ACE Form 107 R: signed/stamped by the responsible DSO of the division you are assigned to.
National ID/Passport

Whenever there is a change of address, please make an appointment and provide the required documents:
