NATO/national CIVILIAN COMPONENT

Documents necessary for SHAPE ID:

Sponsor:

• SF 94: completed electronically, signed, and stamped by your HR Civilian Personnel Department.
• Attestation of a Certificate of Assignment: signed/stamped by the Civilian Branch Office or NMR (National Military Representative)
• National ID/Passport: original, no photocopies
• NATO Security Clearance Certificate
• SF 1: signed/stamped by the responsible Divisional Security Officer (DSO) of the division to which you are assigned.
• ACE Form 107 R: signed/stamped by the responsible DSO of the division to which you are assigned.
  

• SF 94 for each applicant: completed electronically, signed and stamped by your sponsor's HR Civilian Personnel Department.
• National ID/Passport

SHAPE ID Extension

For tour extensions, please make an appointment and provide the documents used for in-processing with updated information in order to extend your SHAPE ID.  All SHAPE IDs associated with this sponsor should extend at the same time.
